The links below will enable you to download important forms that you will need for your child to attend a Play Soccer™ camp. Please make sure you have read the Camp Daily Planner and download the Camp Evaluation Form. The camp planner is very useful and explains the do's and don’ts of our camp week and we need as many evaluations as possible to help to improve your child’s experience.

You must have adobe, print, complete, and mail with a check to the address on the form.
Please remember to include your child’s health form with your application.

It is very important that you print the camp evaluation form and complete it for Friday and take it to camp. We ask you to be honest and fill in every section. For Play Soccer to continue running camps in your town we need your support by enabling us to supply our customers with feedback.

Camp Health Form :- Play_Soccer_Health_Form.pdf
MA health regulations require ever child that comes to a Play Soccer camp must have supplied a health form to head office. This health form can be downloaded here or obtained from your doctor. It will be attached to your child’s application for the start of camp and shown to the towns Health Director. If you do not send one in, your child can not come to camp.
