PLAY SOCCER Terms and Conditions

"PLAY SOCCER camps must comply with regulations of the Massachusetts Department of Public Health (105CMR 430.000) and be licensed by the local Board of Health. Information on 105CMR 430.000 can be obtained at 617-983-6761."

Parents may request copies of background checks, health care and discipline policies as well as procedures for filing grievances as per 105CMR 430.00

By registering for a PLAY SOCCER camp or clinic, our customers agree to conditions, which include but may not be limited to, the following:

Permission	Please submit a permission slip if someone other than the parent or guardian is picking up the camper. I give my consent for my child to be photographed or filmed while participating in camp activities and for the resulting images to be used by Play Soccer™ for promotional purposes.
Campers who miss days	If a camper will miss a session, please contact the camp coordinator prior to the beginning of the camp day.
Illness and Injury	Campers who are unable to participate during a portion of the day will be treated and supervised. Parent/Guardians/Emergency contacts will be contacted in case of more serious illness or injuries. An ambulance will be contacted for campers with injuries or illnesses needing hospitalization.
Lost Property	Lost equipment and personal belongings are not the responsibility of the camp.
Discipline	If my child disobeys camp rules, I agree that may child may be asked to leave the camp with no refund of camp fees.
Refunds	If a camp registration is cancelled more than 21 days prior to camp, a 50% refund minus a $25 administration fee will be issued. If registration is cancelled less than 7 days prior to camp no refund will be issued. No-shows at camp are non-refundable. Cancellations for medical reasons with a valid doctor’s note will be given a full refund minus a $25 administration fee. All refunds for summer camps will be issued after September 1st. Returned checks will be charged a $25 transaction fee.
Camp Status	PLAY SOCCER™ reserves the right to alter the status of any of its camps without prior notice. This may include, but not be limited to, the postponement of full day sessions due to insufficient registrations.
Disclaimer	I hereby release PLAY SOCCER™ CAMPS from any and all claims and liability of any kind of personal injury or property damage due to participation in this camp. I also understand that my child may be photographed during the week of camp, and that these photos may be used for promotional materials. I certify that my child is in good health and is able to participate in physical activities, including soccer. In the event of illness or injury, I grant permission to take appropriate action for my child’s health and safety and to obtain any necessary medical assistance. I will be fully responsible for all medical expenses incurred by my child while attending the camp. Written authorization to administer prescribed medications must be signed by the parent or guardian. This medication must be kept and administered by a health supervisor.
