  Please read through the form thoroughly before filling it out. Answer questions as honestly and fully as possible and attach a photograph in the designated space. We need a contact address and phone number . An e-mail address is essential. 
  
  Make sure you have enclosed all relevant information. We are unable to offer you an interview if your application is incomplete.

• Submit three references

It is important that you print 3 copies of both the reference information sheet and also the reference form. You then give a copy of each to the individual acting as your referee.

Please ensure your name is printed on the top of each sheet. Referees do not necessarily have to have a coaching or sports-related background, but this may help your application. We do not accept references from relatives or close friends.

Please either send us each reference in a sealed envelope, along with your application or ensure each referee mails his or her reference to the address above.
